Here are the steps:

1.  Contact Glenn Daily at gdaily@whatsmypolicyworth.com or 212-426-6265.

2.  We will send you a list of documents that we need. We will also send you a list  of the

       assumptions that you can specify if you wish (or you can let us choose assumptions

       for the first report).

3.  Payment is due when we have the required documents and are ready to prepare the report.
